Abstract :
Indonesia is one among those developing countries that have begun this paradigm shift, that is, shifting from complication management to prevention of complications. This approach is important for Indonesia, because most deliveries still take place in the woman’s home and precious time is lost whenever a woman must be referred to a center for complication management, this paradigm shift to prevention of complication can reduce maternal mortality. If the village-based midwife is trained to prevent complications, to detect and immediately manage any problems in its earliest stages, and to refer as soon as possible, the severity of complications will be reduced and mothers’ lives will be saved. OBJECTIVE: To review the Normal Delivery Training in Indonesia. METHODOLOGY: Systematic review of the literature (Cochrane , WHO Reproductive Health Library 7 , JHPIEGO Strategy Paper) RESULTS: Since 1998 Normal Delivery Training in Indonesia was changed . There was a paradigma shift from from watching for and then treating any complications to proactively preventing complications from ever occurring . Most of the paragdima shift based on systematical review of evidence , and some by research or strategy paper. Preventive practices taught in this training include: a. Avoidance of any unnecessary and/or harmful procedures, such as, those that can slow down delivery progress, increase infection, cause bleeding, worsen neonatal asphyxia, and/or worsen hypothermia in the newborn. Specifically, procedures to avoid are keeping mothers in the supine position, doing routine urine catheterization, doing a routine episiotomy, deep suctioning of the newborn, and bathing the newborn immediately after birth.b. Episiotomy for vaginal birth. c. Position for women during second stage of labor. d. RHL Implementation Aid : Better Births Initiative. e. Active Management of third Stage of Labor. f. Caregiver support for women during childbirth. g. Immediate tactile stimulation and drying and warming of the newborn baby to promote respirations and prevent hypothermia. h. Competency-Based Approach to training CONCLUSION : The clinical skills taught in this Normal Delivery Training (Competency Based Training) can be used as a standard of delivery care for all normal labors . The care provider can be a rural midwife or a birth attendant who provides childbirth care at home or one who works on a team as either a doctor or a midwife in hospitals. All the steps in this training represent the standards of care for any normal delivery.
