Abstract :
Bacterial contamination in blood produts, especially Thrombocyte Consentrate (TC) still remains a problem in the world with potentialclinical consequences from mild symptom to death. Presently, Blood Banks all over the world keep their TC product in careful mannersince even extremely small numbes of bacteria can multiply to vast under storage at 22–20° C and dangerous levels during theirstorage period. In addition, the demands on TC were increasing, some of which were used mostly for immunosuppressive patients aswell as for multiple transfusion. TC product is also called an indicator for other products such as Whole Blood (WB), Pack Red Cell(PRC) etc. In Indonesia, the reports of bacterial contaminations are minimal. The objective of this study was to discover the possibilityof bacterial contamination in TC product at Red Cross Blood Donor Unit (BDU) in Surabaya. BacT/ALLERT equipment was usedto culture and detect the samples for aerobic and anaerobic bacteria. Therefore, whenever the bacterial contamination on TC turnedpositive, precautions were taken during the next manufacturing process. This is due to the risk contained in TC product as well as itsrole as an indicator of other products. WB products and donor blood were also cultured and served as a comparison and to analyzeany source in which possible contaminations take place. The species of detected bacteria was identified using VITEK-2, which waspreviously examined for Gram positive and negative bacteria using Direct Smear. The result of this study showed 3 samples of TCpositively identified as Staphylococcus hominis and Propionibacterium acne where as WB product showed no contamination. Foursamples of TC donor blood were positively identified as Sphingomonas paucibilis and Propionibacterium acne and 4 samples of WBdonor blood were positively identified as Prophymonas assaccharolitica and Fusobacterium varium. The analysis of this study concludedthat bacterial contamination can occur in platelet products with prevalence rates 5%, and 6.67% in blood donors where as in WBproduct there were no contamination found. The bacterial contaminations in donor blood were not followed by the contaminationsin blood products derived from the same donor.The species of bacteria were commensal and less or non pathogenic However, thepresence of the non pathogenic bacteria is a subject as a precaution.The critical point from which contamination takes place includethe manufacturing of TC, phlebotomy process and probable subclinical bacteremia donor.
