Abstract :

Di glomerulonefritis terjadi inflamasi: intraglomerulus, proliferasi sel dan hematuria. Hematuria ditandai adanya > tiga (3)eritrosit/lapang pandang besar di air kemih, yang menunjukkan proses patologis di ginjal ataupun saluran kemih. Gabungankerusakan mekanik membran eritrosit selama melintasi membran basal glomerulus yang rusak diikuti oleh kerusakan osmotikselama melintasi tubulus ginjal dalam suasana hipotonik yang menyebabkan perubahan bentuk menjadi dismorfik. Eritrosit yangterperangkap dalam protein Tamm-Horsfall akan membentuk silinder eritrosit. Eritrosit dismorfik dan atau silinder eritrosit di airkemih menandakan adanya hematuria glomerulus. Di air kemih ada berbagai bentuk eritrosit dismorfik. Akantosit (sel G1) dikatakansebagai bentuk yang khas untuk hematuria glomerulus. Pemeriksaan sedimen air kemih ini dapat dilakukan secara natif ataupunmenggunakan alat analisis air kemih otomatis.
