Dengue serotypes distribution in surabaya in the year 2012

Abstract :

The characteristics of epidemic dengue often presented as periods of hyperendemicity or as the co-circulation of multiple dengue serotypes. Surabaya is an endemic city for Dengue virus (DENV) transmission. Previous study of DENV distribution in 2008-2009 revealed the predominance of DENV-2. DENV serotypes distribution is known to be dynamic and serotype predominance may change through time. This study aims to determine and follow the circulation of DENV serotype in Surabaya in 2012. We recruited 154 dengue-suspected patients attending Dr. Soetomo Hospital during February until August 2012. Dengue cases were confirmed by IgG and IgM serology tests and NS1 antigen detection. Serologically-positive samples were further analyzed using two-steps reverse transcriptase-polymerase chain reaction (RT-PCR) and viruses were isolated by propagation in C6/36 mosquito cell line. Seventy one cases (46.1%) were detected as DENV positive infection. Serotyping revealed that 61 samples have monotypic infection with one of all four of DENV serotypes and 10 samples have mix-infections. Overall serotyping result observed the predominance of DENV-1 (60.56%). Our result revealed the circulation of all four serotypes of DENV and the presence of serotype exchange in Surabaya in 2012. Annual change of predominant serotype and the presence of multiple infections may play an important role in the transmission of dengue infection. This information is valuable to dengue surveillance in the region. Therefore, the laboratory diagnosis of DENV serotype should be routinely performed to follow the dynamic of dengue disease.      Ciri epidemiologis penyebaran penyakit dengue sering ditandai dengan terjadinya hiperendemisitas atau kemunculan beragam serotipe dengue secara bersamaan. Surabaya merupakan kota endemis untuk infeksi virus dengue. Penelitian terdahulu pada tahun 2005-2009 menunjukan dominasi virus dengue DEN-2. Dominasi serotipe virus dengue di suatu tempat adalah dinamis dan pergantian dominasi terjadi sepanjang waktu. Penelitian ini bertujuan untuk mengetahui dan menentukan peredaran serotipe dengue di Surabaya pada tahun 2012. Sejumlah 154 sampel demam dengue (DD) dan demam berdarah dengue (DBD) dikumpulkan mulai bulan Februari sampai Agustus 2012 di RSUD Dr Soetomo Surabaya. Kasus-kasus dengue dideteksi secara serologi dengan pemeriksaan antibodi IgG dan IgM antidengue dan antigen NS1 ELISA dari Panbio. Sampel-sampel hasil positif dilanjutkan dianalisa dengan reverse-trancriptase polymerase chain reaction (RT-PCR) untuk menentukan serotipe dengue. Sampel tersebut juga dikultur untuk mengisolasi virus di sel nyamuk C6/36. Dari sampel yang diperiksa, didapatkan sebanyak 71 sampel (46,10%) positif. Dari 71 sampel, 61 di antaranya terinfeksi serotipe tunggal dengan berbagai serotipe yang berbeda namun didominasi oleh DEN-1. Sedangkan 10 sampel terdeteksi dengan infeksi ganda. Hasil serotyping menunjukkan terjadinya pergeseran serotipe virus dengue. Keseluruhan hasil di atas menunjukkan bahwa keempat serotipe virus dengue dijumpai di Surabaya. Perubahan dominasi serotipe serta keberadaan infeksi ganda setiap tahun memegang peran penting dari penyebaran infeksi. Keterangan ini sangat bermanfaat untuk tatanan dan diagnosis laboratorik serotipe dengue sehingga distribusi serotipe tersebut sebaiknya diteliti secara berkala.
