Flaming cells in multiple myeloma

Abstract :

  Multiple myeloma is a type of cancer on plasma cells which are system of immune  cells in bone marrow that produce antibodies. A 47 years old man precented with an excruciatingly painfull bone lytic lesion acompanied with compressive fracture in his Thorakal XII and first Lumbar vertebral body since a week ago. A complete blood count on admission showed anemia normocytic normocrom with hemoglobin content of 5.3 mg/dL. The blood smear revealed clumping of red blood cells to bound “Rouleaux formations”. Serum protein electrophoresis showed specific evidence of a M-spike. Bence-Jones proteinuria was positive and serum kreatinin arised 2.44 mg/dL. The bone marrow aspiration contained 45% plasma cells, many of which exhibited the morphology of flaming cells with an eccentric nucleus and violaceous cytoplasm. Plasma cells varied in size and shape and included flaming cells and myeloma cells. The patient  was diagnosed as having flaming cells in multiple myeloma stage IIIB.     Multipel myeloma adalah tipe kanker sel plasma yang ditandai dengan adanya produksi antibodi berlebihan dari sel-sel sistim imun sumsum tulang. Seorang laki-laki A 47 tahun dengan nyeri hebat disertai fraktur kompresi tulang vertebra torakal XII dan lumbal I dialami sejak satu minggu yang lalu. Pemeriksaan darah lengkap menunjukkan anemia normositik normokrom dengan Hb 5,3 mg/dL. Ditemukan bentuk Rouleaux pada hapusan darah tepi, M-spike pada Serum protein electrophoresis, proteinuria Bence-Jones positif dan kreatinin serum meningkat menjadi 2,44 mg/dL. Aspirasi sumsum tulang menunjukkan 45% sel plasma, beberapa diantaranya ditemukan bentuk flaming cells dengan inti aneh (eksentrik) dan sitoplasma merah keunguan. Ukuran dan bentuk sel bervariasi termasuk flaming cells dan sel myeloma. Oleh karena itu, pasien ini didiagnosis dengan flaming cells pada Multipel myeloma stadium IIIB.
