Immature platelet fraction (ipf) and thrombopoietin in liver cirrhosis

Abstract :

  Liver cirrhosis remains a major clinical problem worldwide when associated with significant morbidity and mortality due to its complications. The presence of liver cirrhosis state affects the production of TPO influencing the process of thrombopoiesis. The thrombopoiesis activity can be described by the Immature Platelet Fraction (IPF) value which is young platelets. The immature Platelet fraction value increases when platelet production enhances as well, on the contrary when the production declines,  the IPF value is also decreased. This study was performed by cross-sectional method using 31 subject samples suffering from liver cirrhosis, consisting of Child Pugh score class A 2 samples (6.4%), Child Pugh score class B 9 samples (29%) and Child Pugh score class C 20 samples (64.6%). The examination of TPO levels was done by ELISA method using Humans TPO QuantikineR, the IPF value was examined using Sysmex XE-2100 Hematology Analyzer. The thrombopoietin serum levels in the samples ranged from 23.5 to 96.6 pg/mL with a mean of 45.1pg/mL. The immature Platelet Fraction values ​​varied from 1.7% to 19.1% with a mean of 6.7%. From the statistical analysis, the levels of TPO and IPF at various degrees of the disease severity were not significantly different. There was no significant correlation between the TPO level and​​ IPF value, r = 0.038, p = 0.837. There was no significant difference  between the TPO level and the IPF value  in the splenomegaly and nonsplenomegaly state. In conclusion, based on this study no significant correlation was found between the IPF value with thrombopoietin serum levels, as well as the IPF and thrombopoietin levels, and there was no association with the disease severity.     Sirosis hati masih merupakan masalah klinis utama di seluruh dunia bila dihubungkan  dengan angka kesakitan dan angka kematian yang bermakna karena komplikasi yang ditimbulkan. Sirosis hati mempengaruhi hasilan trombopoietin (TPO), sehingga akan mempengaruhi proses trombopoiesis. Aktivitas trombopoiesis dapat digambarkan dengan Immature Platelet Fraction (IPF) yang merupakan trombosit muda. Nilai IPF meningkat bila hasilan trombosit meningkat, sedangkan bila proses hasilan menurun nilai IPF juga  merendah. Penelitian dilakukan dengan metode potong silang dengan sampel penelitian sebanyak 31 orang penderita sirosis hati, yang terdiri dari sampel Child A 2 (6,4%), Child B 9 sampel (29%) dan Child C 20 (64,6%). Pemeriksaan kadar TPO dengan metode ELISA menggunakan reagen QuantikineR Human TPO, untuk pemeriksaan nilai IPF menggunakan alat analisis darah Sysmex XE-2100. Kadar TPO serum di sampel berkisar 23,5–96,6 pg/mL dengan rerata 45,1 pg/mL. Nilai IPF beragam dari yang terendah 1,7% sampai sebesar 19,1% dengan rerata 6,7%. Dari analisis statistik, kadar TPO dan IPF di berbagai derajat keparahan penyakit tidak berbeda secara bermakna. Bila kadar TPO dihubungkan dengan nilai IPF, tidak didapatkan kenasaban yang bermakna di antara keduanya dengan r=0,038;p=0,837. Perbedaan yang bermakna tidak ditemukan antara kadar TPO dan nilai IPF di kelompok splenomegali dan nonsplenomegali. Kesimpulan yang didasari kajian ini, tidak didapatkan kenasaban yang bermakna antara nilai IPF dan kadar serum trombopoietin, serta nilai IPF dan kadar trombopoietin tidak berhubungan dengan derajat keparahan penyakit.
