Abstract :

About 70% of all nosocomial infections occur in surgical patients. In open fracture, there is contact with the environment or unsterile body compartment, so that bacterial contamination may occur and cause infection. Besides debridement, prophylactic antibiotics have been used as a standard procedure in the open fracture management. This procedure may cause antibiotics resistance leading to increase the number of infections. The aim of this retrospective study was to know the characteristics, microbial pattern, and sensitivity of 35 cultures and sensitivity test of open fracture patients in Orthopaedics Department of Wahidin Sudirohusodo Hospital during the period of June 2009–June 2010. The most common bacteria found were Proteus mirabilis (26%), Klebsiella pneumonia (14. 8%), and Providencia alcalifaciens (14%). The sensitivity test for antimicrobials showed that most of the 19 antimicrobials, were resistant. The sensitive antimicrobial is Meropenem.     Sekitar 70% dari seluruh infeksi nosokomial dilaporkan terjadi di penderita yang menjalani pembedahan. Dalam kasus patah tulang terbuka terjadi hubungan dengan dunia luar atau rongga tubuh yang tidak steril, sehingga mudah terjadi pencemaran kuman dan dapat menyebabkan komplikasi infeksi. Di samping pemotongan jaringan pengikat/penekan (debridement), penggunaan antibiotik profilaksis juga telah menjadi tatalangkah baku penanganan patah tulang terbuka. Hal ini memungkinkan resistensi antibiotik terjadi, dengan akibat angka infeksi meningkat. Penelitian ini dilakukan dengan retrospektif, bertujuan untuk mengetahui jenis kuman, dan kepekaan terhadap 35 hasil periksaan kultur dan uji kepekaan dalam nanah (pus) penderita patah tulang terbuka yang dirawat di Bagian Ortopedi RS Wahidin Sudirohusodo Makassar dalam masa waktu Juni 2009–Juni 2010. Hasil kultur kuman menunjukkan bahwa jenis yang terbanyak adalah Proteus mirabilis (26%), Klebsiella pneumonia (14,8%), dan Providencia alcalifaciens (14%). Hasil uji kepekaan terhadap antibiotik menunjukkan jumlah prosentase resistensi yang tinggi, yakni di atas 70%. Antibiotik yang masih peka hanya Meropenem.
