The role for clinical pathologist in hospital accreditation

Abstract :

The hospital accreditation of 2012 version assessment, performed through the evaluation of the implementation of hospital accreditation standards. It’s consisting of four (4) standards groups. The laboratory service is one component of the Assessment of Patients (AOP) chapters. The role of Clinical Pathologist in this accreditation can be internal as well as external. In the laboratory services standard, there are 13 sections each with element of judgment. Some of the challenges are still encountered in the meeting for the accreditation standards include the need for an agreement between the Clinical Pathologist and the Physician, perhaps even the role of the Indonesian Association of Clinical Pathologist and Laboratory Medicine (IACPLM) to determine the values of which one has the critical value, that should be reported by the laboratory to physician. In addition, most laboratories currently in Indonesia do not have a reference value corresponding to the population and geography. This would be the task of the specialist (us) together/IACPLM to be able to formulate the reference value or discuss it with the Hospital Accreditation Committee in order to obtain the same perception of the reference value. But considering the determination of the reference value is not a simple matter and this will require cost as well. Another challenge is controlling the laboratory equipment used in wards, such as Point of Care Testing (POCT) devices. Most POCT is used in wards is not provided by the laboratory. The hospitals should regularly receive reports and review the quality control of the reference laboratory. But so far, most of the reference laboratories do not report the results of quality control to the hospital laboratory. In one assessment standard, stated that the hospitals should have access to contact an expert of diagnostics specialist such concerned to parasitological, virological, or toxicological. The question is whether the Clinical Pathologist can not be regarded as an expert in parasitological and virological diagnostics?   Penilaian akreditasi rumah sakit versi tahun 2012, dilakukan melalui penilaian penerapan bakuan yang terkait akreditasi rumah sakit yang terdiri dari empat (4) kelompok. Pelayanan laboratorik merupakan salah satu komponen di bab pengajuan/Asesmen Pasien (AP). Peran Dokter Spesialis Patologi Klinik (DSPK) dalam akreditasi ini dapat bersifat internal maupun eksternal. Dalam bakuan layanan laboratorik ini, terdapat 13 bagian dengan unsur masing-masing penilaian. Beberapa tantangan yang masih dihadapi dalam pemenuhan bakuan akreditasi ini di antaranya adalah diperlukan kesepakatan antara DSPK dan Dokter Penanggung Jawab Pasien (DPJP), bahkan mungkin peran PDS PatKLin mengenai nilai manakah yang merupakan nilai gawat, yang perlu segera dilaporkan oleh penanggung jawab di laboratorium kepada DPJP. Di samping itu, saat ini sebagian besar laboratorium di Indonesia belum memiliki nilai rujukan yang sesuai dengan populasi dan geografi masing-masing. Hal tersebut diperkirakan menjadi tugas dari para pakar bersama/PDS PatKLin untuk dapat merumuskan nilai rujukan tersebut atau mendiskusikannya dengan Komite Akreditasi Rumah Sakit (KARS) agar diperoleh pendapat yang sama tentang nilai rujukannya. Mengingat penentuan nilai rujukan bukan merupakan hal yang sederhana dan memerlukan biaya yang tidak sedikit. Tantangan lain yang dihadapi adalah dalam mengawasi peralatan laboratorium yang digunakan di ruangan lain, seperti misalnya alat Point Of Care Testing (POCT), karena kebanyakan POCT yang digunakan bukanlah yang disediakan oleh laboratorium. Rumah sakit hendaknya secara teratur menerima laporan dan menelaah pengawasan mutu dari laboratorium rujukan. Namun selama ini, kebanyakan laboratorium rujukan tersebut tidak melaporkan kendalian mutunya kepada penanggung jawab laboratorium rumah sakit. Dalam salah satu bakuan penilaian, dinyatakan bahwa rumah sakit harus menetapkan ahli dalam bidang diagnostik kepakaran seperti yang terkait parasitologi, virologi, atau toksikologi. Muncul pertanyaan dalam pikiran penulis apakah seorang DSPK tidak dapat dianggap sebagai ahli dalam diagnostik yang terkait parasitologi dan virologi?
