The role of γ-interferron gene (ifng) polymorphism in phenotype histology lupus nephritis

Abstract :
Lupus Nephritis (LN) is a serious complication of Systemic Lupus Erythematosus (SLE) with the development of end stage renal disease in 10-70% patients within 5 years. The condition is classified into 6 different classes according WHO criteria. Several studies showed that there were significant clinical manifestation differences between class III, IV and class V LN. It has been suggested that the class differences of LN was related to the cytokines balance and genetic factor. The objective of this study was to determine the role of γ-Interferron gene (IFNG) polymorphism in the class differences of LN. The study was conducted in 40 female SLE patients at the Dr Saiful Anwar Hospital, Malang. Histologic phenotypes classification was based on World Health Organization (WHO) criteria (1995). Microsatelite polymorphism within the first intron of the IFNγ gene on chromosome 12q24.1 was performed by DNA sequencing. The allele difference between LN classes and healthy controle were analysed by Chi-square, the risk of LN in patients with certain IFNG allele was calculated using Odds Ratio The result showed that the frequency of IFNG 112 allele were higher in SLE patients compared with healthy controls (succeptible allele) and the risk to have class V LN in patients with IFNG 112 was 6 times higher compared with patients without these allele There is an association between IFNG polymorphism with the LN classes. Nefritis Lupus (NL) merupakan komplikasi serius dari Lupus Eritematosus Sistemik (LES) dengan terjadinya penyakit ginjal tahap akhir pada 10-70% pasien dalam 5 tahun. NL diklasifikasikan dalam 6 kelas menurut kriteria WHO. Beberapa penelitian menunjukkan adanya perbedaan gambaran klinis di antara NL kelas III, IV dan V. Diduga terjadinya perbedaan kelas tersebut berkaitan dengan keseimbangan sitokin dan faktor genetik. Tujuan penelitian ini adalah mengetahui peran polimorfisme gen Interferon-γ (IFNG) pada perbedaan kelas NL. Penelitian dilakukan pada 40 pasien LES wanita di Rumah Sakit Dr Saiful Anwar Malang. Klasifikasi fenotip[ik histologi didasarkan pada kriteria WHO. Polimorfisme mikrosatelit intron pertama gen IFN-γ pada kromosom 12q24.1 ditentukan melalui DNA sequencin. Perbedaan alel IFNG di antara kelas NL dan kontrol sehat dianalisa dengan Chi-square, resiko timbulnya NL pada pasien dengan alel IFNG tertentu dihitung dengan Rasio Odds. Hasil penelitian ini menunjukkan bahwa frekuensi elel IFNG 112 nyata lebih tinggi pada pasien LES dibandingkan dengan kontrol sehat (alel kerentanan). Resiko timbulnya NL kelas V pada pasien dengan alel IFNG 112 adalah 6 kali lebih tinggi dibandingkan pasien yang tidak membawa alel tersebut. Terdapat hubungan antara polimorfisme IFNG dengan perbedaan kelas NL.
