Utilizing burden of diseases for setting priorities of health resource allocation

Abstract :

Valuing population health status into one single measurement has been through a long history of effort. This article aims at elaborating the use of burden of disease measurement: Disability Adjusted Life Years (DALY lost) as a tool for setting priorities of health resources allocation for health service interventions policies. Epidemiologic measurements currently available e.g. mortality, morbidity and disability rates -which can only demonstrate the intensity of event- cannot present a comprehensive description of community health status. Summary Measure of Population Health (SMPH) i.e. DALY and Disability Adjusted Life Expectancy (DALE) which combines mortality, morbidity and disability into one measurement, can be used for describing population health status and also for health policy setting priorities. DALY itself is able to show population burden of diseases based on specific category, thus, SMPH is appropriate to set problem priorities. However, the Cost Effectiveness Analysis (CEA) which calculates the cost averted can helpfully provide valid information in setting intervention priorities. In real practice of priority setting, it should be acknowledged that the process of priority setting is actually a political process requiring consensus amongst stakeholders.
