Fibrinogen improvement in niddm with peripheral occlusive arterial disease after pentoxifylline treatment

      Abstract :

      Hyperfibrinogenemia is one of traditional cardiovascular risk factors. Elevated fibrinogen level plays an important role in the mechanism of cardiovascular events. Fibrinogen is an important determinant for the platelet aggregation and plasma viscosity, and also has strong predictive value for cardiovascular event; moreover, fibrinogen is mortality predictor for patients with coronary artery diseases. Hence, any effort to lower fibrinogen will be an important mean in the management of cardiovascular diseases. Material and Methods. Thirty four NIDDM patients with peripheral occlusive arterial disease (POAD) who were outpatients of Endocrinology clinic Dr Soetomo Hospital, male and female, age 40 – 65 years, who met the inclusion and exclusion criteria were enrolled the study and divided into two arms of treatment, treatment group and control group. All patients received standard treatment of NIDDM, which are B diet, exercise, and oral hypoglycaemic agent Gliclazide. Treatment group received pentoxifylline 400 mg t.i.d for 6 months, while control group received placebo instead of pentoxifylline. Fibrinogen level was measured at baseline, 3 months and 6 months after treatment. Results. Treatment group revealed significant lower fibrinogen level after 3 months and 6 months of treatment. Conclusion. This study disclosed that oral Pentoxifylline for 3 and 6 months significantly improved the fibrinogen plasma level in NIDDM with POAD.
