Psychoneuroimmunology paradigm in diabetes mellitus research

      Abstract :

      Diabetes Mellitus (DM) is a metabolic disease characterized with hyperglicemia caused by secretory inhibition, insulin activity, or both. Based on its etiology, DM can be classified into immunologic reactions-mediated DM, also called Type I Diabetes Mellitus, and uncommon forms of immune-mediated diabetes. Studies relating DM with stress are recently being undertaken. Psychological stressor presumably affects glucose metabolism, leading to the decrease of insulin activity and increase of blood glucose. However, up to now mechanism that correlates stress and DM remains uncertain. This uncertainty deserves an immediate thorough study. Otherwise, the role of psychological stress as a factor related to Diabetes Mellitus remains unknown. Consequently, it may impede development of DM management as a whole. Psychoneuroimmunology has developed from a field of study to a science with distinct paradigm. Psychoneuroimmunology is focused on immune response modulation as a manifestation of immune system response against stressor. The paradigm or reasoning model of psychoneuroimmunology is based on immune response modulation due to stress immune system, while conceptually immune response modulation may increase or decrease. The increase of immune response during atopy may aggravate damages due to immune response modulation itself. If it occurs in beta pancreatic cells, the decrease of insulin production is highly possible. However, empirical evidence obtained from research is needed to confirm it. This article will discuss the paradigm and concept of psychoneuroimmunology that should be noted in Diabetes Mellitus research.
