Thirteen-year experience (1987 – 1999) of the management of chest injury in surabaya, indonesia. a retrospective study*

      Abstract :

      Background: The severity of chest injury patients was observed to be increasing from year beside the quantity of the trauma. Death due to chest injury occurs in one of three time periods: the first period, the second period, the third period. The etiologies of death due to chest injury were injury of the heart, aorta, other large blood vessels; multiple organs associated with significant blood loss; and hemopneumothorax. Objective: To reveal the general features correlated to incidence, etiology, sex, age, severity of the chest injury patients and the way to manage it. Material and Method: The retrospective study of the 1906 chest injury patients treated during the period of 1987-1999 (13 years) at the Surgical Emergency Department Dr Soetomo Hospital Surabaya provided comprehensive data concerning chest injuries which collected and analyzed. Results: A total number of 358043 trauma patients, 1906 (0.53%) of them suffered from chest injuries, were treated in the Surgical Emergency Department of the Dr. Soetomo Hospital, in the period of 1987-1999 (13 years). Of the 1906, 1642 (84.15%) were male and 264 (13.85%) were female. Their ages ranged from 4.5 to 90 years. The youngest got a penetrating air rifle injury, while the oldest was a victim of a traffic accident. The etiologies were traffic accident 1341 patients (70.36%), assault in 366 patients (19.20%), occupation accident in 150 patients (7.87%%) and, home accident in 49 patients (2.57%%). Most of them, 1125 (59.02%) were conservatively treated. Intra-thoracic drainage was 702 patients (36.84%) and explorative thoracotomy was 79 patients (4.14%). There were 40 hospital deaths (2.09 %), all with severe multi-trauma in which one case was caused by penetrating injury. Conclusions: The majority of the chest injury patients presented at the department of surgery Dr. Soetomo Hospital Surabaya were treated conservatively, and only 79 (4.14 %) patients were treated by emergency thoracotomy. The overall mortality rate of chest injury was 2.09 %.
