Abstract :

Hazard identification, risk assessment and control of hazards are part of risk management which is the basis of the application of management system of safety and occupational health. The result is used to determine and implement appropriate control in order to prevent accidents at work and occupational diseases. This study is aimed at identifying the potential hazards using Job Safety Analysis (JSA) method, performing risk assessment, and determining the efforts to control the danger in CV. Duta Paving Mandiri, Pasuruan. The research design used in this study is cross sectional researchusing observation and descriptive analysis. The object of this study is potential hazards, risk assessment and control of hazards in the Production Division of CV. Duta Paving Mandiri, Pasuruan. This study was conducted in March-April 2014 and usedboth primary and secondary data. CV. Duta Paving Mandiri, Pasuruan has not properly implemented risk management activities. However, the company has performed non formal risk assessment. According to the result of the study, 68 potential dangers and 130 hazard risks were found in the company. Among those risks, 70 findings had low risk, while the rest had midle risk. No high risk was found in the study. Implementation of hazard identification and risk assessment should be carried out by qualified team in order to make the results more accurate. Moreover, the company must ensure that the current control is properly operated and well maintained.
