Association between refractive changes and blood glucose changes in diabetic mellitus type 2 patient

Abstract :

Purpose : To evaluate association between Refractive Changes and Blood Glucose Changes in Diabetic Mellitus Type 2 Patient. Method : Twenty eyes of patients with diabetes mellitus type 2 were included in this prospective study. During the observation period, patients underwent general ophthalmological examination including refractive measurement and blood glucose level at first admission then second measurement was performed when patient controlled. Measurements at baseline and during maximal hyperglycemia were analyzed. Results : A refractive change occurred in all patients who had blood glucose changes. The patient had DM 1 – 4 years (mean 2.5 years) and HbA1C 6.2 % - 7.53 % (mean 7.03 %). On first admission refractive measurement – 4.00 D until + 4.50 D (– 0.44 ± 2.51) and when controlled – 3.50 D until + 3.50 D ( – 0.39 ± 1.99). Blood glucose concentration 105 – 361 mg/dL (186.73 ± 61.94) and when controlled 112 – 492 mg/dL (229.31 ± 92.41). Refractive changes was 4.00 D until 1.25 D (– 0.04 ± 1.46) and blood glucose concentration changes was – 63 until 131 mg/dL (44.15 ± 48.49). There was a negative correlation between refractive changes and blood glucose changes (p< 0.05) and there were refractive changes – 1.007 D every 100 mg/dL blood glucose changes. Conclusion : There was a negative correlation between refractive changes and blood glucose changes in patient with diabetic mellitus type 2. This finding could provide the refractive changes that are often observed in patient with diabetic mellitus also a reduction of refractive index in intraocular tissues, especially in lens, appears to be responsible for this refractive change.
