Risk difference of hypertension occurrence based on smoking pattern

Abstract :

Cigarette smoking is a main risk faktor for cardiovascular morbidity and mortality; one of them is hypertension, researches on hypertension still shw no significant relationship. Beside that, hypertaension cause severe complications such as stroke, coronary heart diseasea, occular and renal disorders. The purpose of research is study risk difference of hypertension occurrence based on smoking frquency, number of cigarette a day, type of cigerette, way of smoking, age intiation of smoking, and duration of smoking.The study was an observational research with case control design. The study was condubted in Haji Hospital, Surabaya city. The subyect was recruited from patients who attended to the hospital. As a case is with inclusiom criterias are hypertension patient and smoke, do’t have any diseases related to hypertension such as diabetes melitus, chronic renal failure, to participate in this study voluntary with informed consent As a control is a smoke without primary hypertension and secondary; and to participate in this study voluntary with informed consent. The number of case are 75 patients and of control are 75 patients.Bivariate analysis show that a risk difference of hypertension occurence based on age initiation of smoking at 16 – 18 years old with risk 3,39 times (95% Cinfidence Interval 1,41 – 8,28) more than age of initiation of smoking at 19 – 35 years old. Duration of smoking for 18-35 years show a risk difference of hypertension occurence with risk 5,21 times (95% CI 2,02 – 13,68) and duration of smoking for 18 – 32 years show it risk 3,78 times (95% CI 1,51 – 9,58). Multivariate analysis show that there is a risk difference of hypertension occurrence based on age of intiation at 16 – 18 years old with risk 4,81 times (95% CI 1,79 – 12,94) more than age of intiation at 19 – 35 years old. The number of cigarettes a day is 10-20 pieces, it show a risk differrence of hypertension occurrence with risk 3,02 times (95% CI 1,19-7,63) more than number of cigarete is < 10 pieces a day.Resarch resul show; Satar to smoking at 16 – 18 years old, number of cigarettes is 10–20 piece a dya and length of smoking more than 17 tahun, thay show a risk difference of hypertension occurrence. Hypertension cases can be decreased until more than 65% if the risk factor can be eliminated.
