Economic loss of toraja community due to disability and premature death in south sulawesi province, indonesia

Abstract :
“Health is an investment” has been proven by many studies, therefore it should be managed properly to prevent a ”loss generation” and we must invest for the future generation. In 1993 WHO (World Health Organization) issued a new method for more effective and efficient resources allocation by using DALY (Disability Adjusted Life Years) method.The aim of the study was to find and analyze the total economic loss of the Toraja community due to disability and premature death. This observational study was a logical induction in health economics to detect the socio-cultural factors contributing to the economic loss of the Toraja community. The observation was done at Tator district, the sample was taken by a proportional random sampling method consisted of 172 respondents interviewed, and 40 informants were in-depth interviewed.The result of the study showed that the direct cost of Household Expenditure for medical expenses was Rp3,498,482,696.00; traditional care expenses for Ma’dampi was Rp608,808,466.0; for Massalusalu was Rp11,095,000.00; and for Ma’ro and Ma’bugi were Rp1,550,000.00. The Government Health Expenditure was Rp5,077,096,378.00. The externalities for the traditional ritual Rambu Tuka was Rp6,943,862,950.00 and for Rambu Solo was Rp279,130,581,750.00. The indirest cost due to sickness and disablility = YLD (Years Lived with Disability) was Rp54.633.015.000.00; premature death before reaching one’s life-expectancy = YLL (Years of Life Lost) was Rp26,044,575,000.00 and the lost cost due to premature death = DALY (Disability Adjusted LifeYears) was Rp80,677,590,000.00. The total economic loss of the Toraja community for the year 2003 was Rp375,949,067,240.00 or 49 times greater than the “Regional Indigenous Income (RII)”. Due to the big economic loss (49 times RII) or 74 times greater than government expenditure in health, the community hand-in-hand with the government should initiate a “Social Health Insurance” which covered the total population without discriminating the rich and the poor. The premium for this intervention should be taken from the traditional activities Rambu Solo and Rambu Tuka.
