The influence of midwives’ need fulfillment on the quality of antenatal care service at tulungagung regency public health centers

Abstract :

The reported coverage of antenatal care (ANC) service at public health centers (PHCs) in Tulungagung regency for the first visit (K1) was 92.7% and for the fourth visit (K4) was 83.37%. This coverage has not reached the national target for K4 (95.0%). It was also unclear, whether the K4 in Tulungagung matched the standard of ANC quality of service criterias i.e. K4 must fulfill the criteria of: antenatal first visit in the first trimester of pregnancy; at least one visit at second trimester and at least twice at third trimester of pregnancy or coded as K4 = 1,1,2. The determinant factors of ANC quality is the fulfillment of midwives’ needs of: a) facilitative supervision and management; b) information, training and development; and c) supplies, equipment and infrastructure, and client’s right . The objective of this study was to find out the relation between midwives’ need fulfillment and client’s right towards the quality of ANC services. The result indicated that the quality of ANC was low as indicated by K4 (only 22,49%) and the facilitative supervision was significantly related to the quality of ANC service, as well as the privacy assurance of client’s care. It was concluded that even though supervision and feedback had been done by PHCs head of and regency health supervisor, the supervision had not matched the need of midwives, resulting in low quality of ANC. It was recommended to develop supervision checklist of ANC, matching the need of midwives for ANC services quality improvement and to provide technical trainings for midwives capability improvement.
