Effect of enteral glutamine supplementation for the low birth weight on gaining weigh pattern and level of faecal secretory iga

Abstract :

Background: Extrauterine growth retardation, immune deficiency is a major problem of low birth weight (LBW). Babies have a biphasic pattern in growing weight loss before after. It is essential for LBW to achieve growth spurt equal to or greater than intrauterine growth, glutamine plays a role in the catabolic phase, stimulates protein synthesis to optimize growth, besides glutamine as fuel for the immune system, especially in the enterocytes of the gut mucosal immune responses. Purpose: To prove the effect of enteral glutamine supplementation on gaining weight pattern and faecal secretory IgA Method: A double blind randomized control trial. Two groups assin as glutamine and placebo group. Glutamine group receives breastfeed, and preterm formula supplementation with glutamine 400 mg/kg/d for 14 d was observed 30 d, placebo supplementation with glucose. Monitoring of return to birth weight, gaining weight velocity and level of faecal secretory IgA. Results: 37 LBW infants (1500-2499 g) were randomly grouped double blind into 18 glutamine group and 19 placebo. Glutamine group had a return to birth weight shorter than placebo (8.1 ± 0.9 vs 11.0 ± 1.6 days) and faster the gaining weight velocity r (20.0 ± 1.8 vs 15.5 ± 2.2 g / kg / day), statistically significant (p = 0.0001). sIgA levels after glutamine supplementation was higher than the placebo group (0.456 ± 0.057 vs 0.376 ± 0.035 mg / g), the difference was statistically significant (p = 0.0001), whereas levels of faecal sIgA of glutamine group 76% higher than placebo. sIgA levels after treatment in each groups of glutamine greater than placebo (0.274 ± 0.024 vs 0.140 ± 0.016 mg / g), was statistically significant (p – 0.0001). Conclusion: Enteral glutamine supplementation in LBW speed return to birth weight, increasing the gaining weight velocity and the levels of faecal secretory IgA.
