Abstract :

One of the issues concerning to the quality of hospital care is the medical records. Dr. Moh.Soewandhie Hospital in Surabaya failed to provide outpatient medical records in accordance to the standard. Therefore, this study aimed to analyze determinant factors in the long-time provision for outpatient medical records at dr.Moh. Soewandhie hospital Surabaya. This was an observational descriptive study and used cross sectional study design. This study used total population as many as 19 employees at outpatient medical record unit that worked in customer service, registration, outpatient medical records searching and medical records distribution. Data obtained through interviews, assessment, observation and documents accordance. Data was analyzed using quantitave method with qualitative method for additional information. There weresix determinant factors, that were employee workload, facilities, the wide for medical record storage, work distance, outpatient medical record storage and job design. These factors were the root of problems in the failure of outpatient medical record provision.
