Abstract :
Background: Thyroid carcinoma is the most common endocrine malignancy with a high mortality rate. The majority of papillary thyroid carcinoma as well differentiated thyroid carcinoma has a good prognosis. Various studies were conducted to study various molecular markers in determining prognosis, one of which is the bcl-2 as a protein that plays a role in the process of apoptosis. Bcl-2 is a proto-oncogen and found in many epithelial malignancies. Excessive expression is believed to prolong the lifetime of the cell and the possibility of chromosomal abnormalities leading to oncogenesis. Objective: Determine whether there is a relationship between the expression of bcl-2 on the incidence of metastases in papillary thyroid carcinoma. Method: An analytical observational cross sectional study was done. Thirty samples of paraffin blocks from the patients with papillary thyroid carcinoma who underwent surgery at Department of Surgery Dr. Soetomo Hospital Surabaya during 2009-2011 were examined immunohistochemistry to detect the bcl-2 expression. Correlation between bcl-2 expression and the occurrence of regional or distant metastases of papillary thyroid carcinoma were analyzed. Results: Out of 30 cases, 21 (70%) showed positive bcl-2expression, and 9 (30%) negative bcl-2 expression. There were 13 (43.33°%) metastases consisted of10 (33.33%) lymph node metastases, 1 (3.33%) distant metastases, and 2 (6.67%) lymph node and distant metastasis. The statistically analysis using chi-square and Fisher's exact test showed that correlation between bcl-2 expression and lymph node metastasis hadp = 0.139 (p > 0.05) and odds ratios 3.5. While the correlation between bcl-2 expression and distant metastases hadp = 1.000 (p > 0.05) with odds ratios of 1.0. Conclusion: There were no significant correlations between the bcl-2 expression and the occurrence of lymph node metastases or distant metastases in papillary thyroid carcinoma.
