Rectus muscles transposition for the treatment of bilateral traumatic abduscens palsy

Abstract :
The purpose of this case report is to report the surgical management of bilateral abduscens palsy due to head trauma. We reportclinical findings of a 45 years old woman presented to our strabismus clinic with severe bilateral paralytic esotropia after head injury8 months ago. The patient complained of severe diplopia and headache. Prism Cover Test examination revealed 75 prism diopter.There were severe limitation on abduction in her both eyes. Worth Four Dot Test revealed diplopia. MRI and CT Scan examination iswithin normal limit. We performed Jensen procedure for transposition the inferior and superior rectus to the lateral rectus and medialrectus recession on her left eye. In her right eye, we performed Modified Hummelscheim procedure for transposition the inferior andsuperior rectus to the lateral rectus and tucking lateral rectus muscle. After underwent these surgical procedure, the patient achievesatisfying results. There is no diplopia and the eye become orthophoric. In conclusions transposition surgery seems to be a suitableprocedure for the treatment of abduscen nerve palsy. In cases of a complete lack of function of an extraocular muscle or in complex,uncommon conditions, which are difficult to treat with any other surgery, muscle transposition seems to be an effective procedure
