Abstract :

Lower urinary tract trauma may be caused by blunt, penetrating or iatrogenic trauma. urinary tract trauma occurs in 10% of all trauma, and 2% of which involves the bladder. pelvic fracture is associated with bladder trauma in 70-97% of cases, but only 5-10% of all pelvic fractures is associated with bladder trauma. of all bladder trauma associated with pelvic fracture, 54-56% are extraperitoneal rupture, 38-40% intraperitoneal and 5-8% are the combination of both intra and extraperitoneal rupture. suspicion of bladder trauma is based on the mechanism of injury, clinical examination and the presence of hematuria in patient with pelvic injury. A quick and accurate diagnosis is necessary in bladder trauma to achieve good result and minimize morbidity to the patient. serious complications may result from delayed diagnosis or inappropriate management due to misdiagnosis, delayed management of multi trauma may cause complex problems. Although cystograpy is the standard procedure to evaluate the bladder rupture with almost 100% sensitivity we report one case of bladder rupture undetected on cystography.
