Ureteral injury from external trauma: missed diagnosis despite extensive initial radiologic investigation

Abstract :
Ureteral injury from external trauma is a rare condition, comprising less than 1% of all urogenital injuries. Patient with ureteral injury generally sustains trauma with high energy, resulting in multiple associated injuries, including those injuries that is rarely happen, such as vertebral column or intestinal injury. In the majority of cases, ureteral injury was detected during exploration. But in the era of non-operative management for blunt abdominal injury, contrast-enhanced helical CT scan remains the goldstandard for the diagnosis of ureteral injury. Nevertheless, delayed or missed diagnosis is still approximately 8-20%, despite application of this modern radiologic technology. We report 1 case of ureteral injury due to blunt abdominal trauma, with multiple concomitant injury. The diagnosis of ureteral injury is missed at presentation, though complete urinary tract imaging had been done. Application of special imaging technique, for example delayed image, is recommended to detect ureteral injury more accurately in highly suspicious case.
