Expression of αvβ3 endometrium after clomiphene citrate, n-acetyl l-cystein, or combination of clomiphene citrate and n-acetyl l-cystein given to polycystic ovarian syndrome case

Abstract :
Hyperandrogenic condition in Polycystic Ovarian Syndrome (PCOS) may affect the endometrium, presenting as the increase in the number of androgen and progesteron receptor in luminal epithelium as well as in endometrial tissue during implantation, which inhibits the expression of integrin avß3, a biological marker to determine endometrial receptivity in pre-implantation period. Clomiphene citrate (CC) is widely used as ovulatory inductor. However, CC has antiestrogenic effect in the endometrium and cervical mucosa. In patients with PCOS, the antioxidant N-Acetyl-L-cysteine (NAC) is able to reduce circulatory insulin and decrease peripheral insulin resistance. This study observed whether there was difference in the expression of integrin avß3 in the endometrium after the administration of clomiphene citrate, combination of clomiphene citrate and NAC, or NAC alone. This was a blind randomized experimental design using Wistar strain Rattus novergicus as PCOS case model with 9 treatment groups. The animals were injected with testosterone propionate, and were administered with CC or combination of clomiphene citrate and NAC, and/or NAC only. It was found that integrin avß3 expression in group receiving NAC therapy was higher compared to that in group receiving placebo, and so was in group receiving combination of CC and NAC compared to that receiving CC only. Integrin avß3 expression in group receiving combined CC and NAC therapy was not proved to be lower compared to that in group receiving NAC only, but it was higher in group receiving NAC therapy compared to that in group receiving CC therapy.
