Abstract :
Corticosteroid and its synthetic analogues play a major role in treating vast kinds of diseases for the last 6 decades, but its psychiatric adverse effects were reported on the same year of its first commercial use. Steroid psychosis refers to a spectrum of symptoms ranging from subtle mood changes to full blown affective syndrome, cognitive impairment and frank psychosis. The general incidence of steroid psychosis is around 6%, and appeared to be dose dependent. It is more common in women than men, due to its underlying disease treated by corticosteroid. Despite the well-known role of HPA axis in cortisol and mood regulation, the pathophysiology of steroid psychosis is still poorly understood. Most reactions resolve within weeks by tapering or discontinuation of steroid. For severe reactions where psychosis remains or tapering was not an option, patient generally responds well with low dosage of antipsychotics. Mood stabilizers were often used as prophylaxis, although further studies are needed. Antidepressants were used in several cases with depressive symptoms, in favor of using SSRIs. Tricyclic agents were not advised due to their effects on exacerbating symptoms. A thorough history taking on possibility of corticosteroid consumption should be obtained by clinician during anamnesis, physical and laboratory examination. Educating patient and family on the possibility of psychiatric adverse effects of corticosteroid are needed for early intervention. Longitudinal studies and clinical trials are needed to better explain many confounding variables and unanswered hypotheses.
