This study analyzed factors that affect young people in the decision to smoke. Methods: This study was an analytic descriptive study using cross-sectional approach. Respondents in this study consisted of 186 students of grade XI, SMAN 16, Surabaya. Final data were analyzed using Spearman's rho correlation test with a positive result if p value < 0.05. Results and Analysis: The results of the study showed that knowledge had p = 0.00 with correlation coefficient of -0.384; attitude p = 0.00 with correlation coefficient -0.294; family behavior p = 0.02 with correlation coefficient 0.188; peer's behavior p = 0.00 with correlation coefficient 0.32; cigarette advertising p = 0.012 with correlation coefficient 0.185; necessity to affiliate p = 0.016 with correlation coefficient 0.176; need for achievement is p = 0.00 with correlation coefficient 0.314. Discussion and Conclusion: The decision to adopt smoking behavior among adolescents is influenced by knowledge of the dangers of smoking, attitudes towards adolescent smoking behavior, family behavior, peer's behavior, exposure to cigarette advertisement, need for affiliation, and need for achievement.
