Abstract :
Soaring health cost has forced health providers to find solutions for an effective and efficient prospective payment. This research aims to formulate Diagnosis Related Group’s of 5 majority diseases with the present value computation at the Inpatient Ward of Surabaya Lung Disease Eradication Clinic (SLDEC). Five majority diseases are Diabetes Mellitus, Pleural Effusion, Acid Fast Bacilli (AFB) +, AFB -, and SKP. A retrospective-prospective study conducted for 2 months at the SLDEC, the research object was secondary data from Medical Records and inpatients administration notes with diagnosis of 5 majority diseases from January2005-April 2007 amounting to 1,211 cases. The results were grouping cases based on: similar diagnoses and differential diagnoses, similar type of rendered services, and similar length of stay and hospital cost. Then, based on this grouping, DRG’s was formulated for 5 majority diseases i.e. Average Length of Stay (ALOS), trim point, cost weight, cost per case, and cost per daycare. The limitations in this research were: no standard diagnosis and therapy, the service tariff was not computed based on unit cost and the non-computerized SLDEC management information system. The conclusion is to ultimately possess a service standard to preserve cost and to secure the quality of healthcare.
