Intraperitoneal adhesion prevention with an intramuscular injection of diphenhydramine perioperative

Abstract :
Background : intraperitoneal adhesions are the most coomon cause of intestinal obstruction. Incidence of intraperitoneal adhesions after laparotomy is still quite high and still an usolved problem. Approximately 55% of patients after laparotomy occures adhesio, with approximately 25% of patients experienced obstruction. Many attempted and tested ways to reduce or prevent the occurence of intraperitoneal adhesions, one ith the administration of anti-hitstamine drugs (Diphenhydramine). Objective : to determine the effect of perioperative intramuscular injection of Dipenhydramine against the occurence of intraperitoneal adhesions after laparotomy in rabbits. Methods : this research is an experimental study with a Randomized post test only control group design with rabbits as an animal subject (Oryctalagus cuniculus) that fit the inclusion criteria and requirements of healthy rabbits. Laarotomy and peritoneal manipulation performed on 22 rabbits as experimental animals which were divided into two groups with each sample number 11. The treament group was performed a perioperaive administration of intramuscular injection of diphenhydramie 30 minutes prior to the manipulation of the peritonesum and for 3 consecutive days, while the control group was not given an intramuscular injection of dipenhydramine perioperative administration. The second laparotomy will be performed 10 days after the first surgery and adhesion assesment was done by using adhesion scoring system of Fiedler which was a modification score of Boyers in 1996. Result : there has been found statistically significant difference with P<0.0001 for the assesment of score of extent area, score of severity, score of tenacity and P=0,002 for the assement of score of inflamation. Conclusion :Intramuscular perioperative injection of diphenhydramine can prevent the occurence of intraperitoneal adhesions after laparotomy in rabbits based on the assement of adhesion scoring system.
