Thymoma dengan anemia aplastik dan occular myasthenia gravis

Abstract :

Abstract Thymoma is tumor from thymus glands. Thymus associated with imunity sistem, several patient with thymus glands tumor have associated with autoimun disease. The disease that can occurs  myasthenia gravis, polymyositis, hypogammaglobulinemia and  cytopenia such as PRCA (Pure Red Cell Aplasia) and aplastic anemia. Case Female, 72 years old, pension nurse, javanese with Mediastinum  anterior tumor (Thymoma) came to DR. Soetomo Hospital with chief complain body weakness. From physical examination we found conjunctiva anemis,  asimetric chest movement with right diminished, decrease tactile fremitus on the right hemithorax, dull percussion on the paracardial right lung. Laboratory: anemia (Hb= 7,65g/dl). Rontgen: Opacity lobulated, clear edge, regular, forming a blunt angle with lung parenchym, projected Vth 5-10 on the right and it seems mediastinum  mass.  CT Scan : Mediastinum tumor right anterior superior about ± 7,37 x 4,44 x 7,2 cm suspect thymoma.  FNAB:  Thymoma mediastinum tumor right anterior superior. In the ward we found that the patient suffered  occular myasthenia gravis. Then the patient got de-bulking operation and the result from PA : Mass from regio mediastinum,  thymoma type B1, pT2N0M0 (stage 2). On 23-10-2012  we consult the patient for radioterapi as adjuvan kemoradioterapi . Then the patient got radioterapi and we send the patient home. Then the patient control regularly to clinic POSA. On 3 des 2012 patient hosptalized again because of  anemia with Hb 8,0 and patient got transfusion. On 21 Desember 2012 patient also hospitalized in interna ward because of anemia with Hb 4,0 and got transfusion, then the patient get bone marrow examination with aplastic anemia as the result Summary Female, 72 years old, pension nurse, javanese with Mediastinum anterior tumor (Thymoma) came to DR. Soetomo Hospital with chief complain body weakness. From physical examination, rontgen, FNAB, laboratory findings we found that the patient suffered thymoma and  in the ward we found patients  occular myasthenia gravis. Then the px got de-bulking operation and the result from PA : Mass from regio mediastinum,  thymoma type B1, pT2N0M0 (stage 2).  The patient got radioterapi as kemoradioterapi adjuvan and we send the patient home. Then the patient control regularly to POSA clinic.  After surgery patient has been hospitalized twice because of anemia and then the patient got bone marrow examination with aplastic anemia as the result.
