Change in total symptom score (tss) dan qol of the allergic rhinitis patients after loratadine therapy

Abstract :

Background:Allergic rhinitis (AR) is one of the diseases of nasal mucosa caused by inflammatory process mediated by Ig E after allergen exposure. The symptoms of AR characterized by rhinorrhoe, nasal obstruction, sneezing and nasal itching which sometimes relieved by itself or with treatment. In some cases the symptoms might be accompanied by eye symptoms (itching, epiphora, with redness of the eye), itching of the ear and throat. AR is a non fatal disease but the severe symptoms might decrease the quality of life (QOL) of the patients. Purpose : to determine if there is a change in total symptom score (TSS) dan QOL of the AR patients after loratadine therapy.  Method:pre experimental study conducted in outpatient department of ORL-HNS in Dr. Soetomo General Hospital during Nopember 2011 to June 2012. Samples collected as consecutive sampling with total samples based on sampling formula count. From those 16 samples, evaluation of the TSS and QOL pre and post loratadine therapy were compared using statistical paired t-test and Pearson correlation test. Result :The research showed that samples were 50% male and  50% were female, with average age samples were 33 years old and the ages range from 19-54 years. The clinical symptoms mostly complained were rhinorrhoe (100%) and sneezing (100%), while domain of the QOL that mostly suffered were nasal symptoms (100%). The most changing symptoms post loratadine therapy were the disappearing of sneezing (50%).  Total QOL were also changing significantly 19.44 (>6) as there were improvements of every domain (the changes varies from 2.06-5.87, >1). The changes of TSS pre and post loratadine therapy was significant (p value =0,000; significant p value <0.5) and there were also significant changes of QOL pre and post loratadine therapy with p value 0.000 (p<0.05). The relationship between the changes of TSS and QOL post loratadine therapy was p=0.000 with r = 0.774, showing strong positive correlation of changes of TSS with QOL. Conclusion :there is a change of TSS and QOL, there is strong positive relationship between changes of TSS and QOL of the AR patient after loratadine  therapy.
