Analysis of the level of rationality of blood tranfusion in emergency surgery serviced in the emergency department soetomo general hospital surabaya

Abstract :
Background: blood transfusion is an act to transfer the blood component or a man’s blood (donor) to the others blood flow (recipient). Tranfusion was done as an act to save lives by replacing blood cell or the loss of blood product caused by hemorrhage. A rational transfusion is a transfusion that gives significant improvement to patients such as hemodynamic improvement and tissue perfusion, with less complication to prevent transfusion reaction that was reached by transfusing blood until the critical indicator was reached in accordance to comorbid or the special condition of patients. Objective: to analyze the rationality of blood transfusion and safety measure of the blood transfusion in emergency department. Design: Observasional descriptive research Methods: this research was to analyze the rationality of blood transfusion and safety measure of the blood transfusion which was performed by Anesthesiology and Reanimation residents on patients that was undergone emergency operation in the operating theater 5th floor of Emergency Department RSU Dr. Sutomo Surabaya based on clinical condition , blood gas analysis, hemoglobin, and hematocrite pre operation and 2 hours after the surgery. Patients that undergone surgery in the emergency department then classified into four class of hemorrhage, based on the amount of hemorrhage that happened, and then the haemodinamic measurement was evaluated. The procedure then repeated again after the surgery with the same parameter, and them evaluated whether the judgement of doing the blood transfusion was rational or not. Results: the data that was collected showed that the blood transfusion was rational enough. Fluid resuscitation that was performed to replace the blood loss was good that no significant difference between the sistolyc and diastolic pressure before and after the blood transfusion with P = 0,08 ( p > 0,05), but significantly different for heart rate parameter with p = 0,01 (p< 0,05). Therefore, the goal of blood transfusion to fix the macro hemodynamic parameter could be reached. Conclusions: the hemoglobin examination with the Sahli method can still be considered to be used as a fast, reasonable, cheap, and précised enough to be a hemoglobin examination on the spor to decide whether the blood transfusion was going to be done or not.
