Diagnostic test of serum glial fibrilary acidic protein for differentiating intracerebral hemorrhage stroke and non hemorrhage / ischemic stroke in acute stroke patients

Abstract :

Background: The rapid differentiation between intracerebral hemorrhage (ICH) and ischemic stroke using biomarker testing would allow cause-spesific management of stroke patient. Head CT Scan  is a gold standart for stroke but its still a rare examination of the area and require a relatively high cost. Glial fibrillary acidic protein (GFAP) is a brain-spesific astrocytic protein which identified as a promising biomarker in acute stroke patients. Objective: This study aimed to assess the sensitivity and specificity serum GFAP for differentiating intracerebral hemorrhage and ischemic stroke in acute stroke patients. Design of study: This is a clinical study, cross sectional analytical, and diagnostic test design. The design are performed on all patients who are diagnosed as acute stroke patients in Dr. Soetomo General Hospital Surabaya from October 2012 to January 2013. Methods: There were 73 subjects who met the inclusion criteria, conducted serum GFAP and head CT Scan, then analyzed by the Chi-square test 2 x 2 tables. Results: The sensitivity, specificity, positive predictive value, negative predictive value, accuracy and likelihood ratio of serum GFAP of head CT scan are 85.7%, 64.4%, 60%, 87.9%, 72.6% and 2.41. Conclusions: serum GFAP have high sensitivity for differentiating intracerebral hemorrhage stroke and ischemic stroke in acute stroke patients.
