Relationship between intraoculi pressure, intracranial pressure and mean arterial pressure on severe brain injury patients

Abstract :

Background: - Objective: to know the correlation between intracranial pressure and intraocular pressure, intraocular pressure and mean arterial pressure in patients with brain injuries. Design: Analytic observasional longitudinal  Methods: This research evaluated the correlation between intraocular pressure, intracranial pressure and mean arterial pressure in patients with brain injuries, admitted to Dr. Soetomo General Hospital, from January 2011 to July 2011. All patients required the intracranial pressure monitoring on clinical grounds. The patient then had tetracaine placed in each eye for anesthesia, and the patient’s bed was placed at 30 degrees to the horizontal for 15 minutes. A handheld tonometer, the Tono-Pen XL (Medtronics Solan, Jacksonville, FL), was then used by the investigator to obtain intraocular pressure. Simultaneous recordings of intracranial pressure, intraocular pressure and mean arterial pressure were performed. Results: This research calculated both the linear correlation coefficient and the Spearman rank-order correlation coefficient for all three relations. This research found significant correlation intraocular pressure  pre and post surgery also intracranial pressure  pre and post surgery. However, the correlation between intraocular pressure and intracranial pressure, as well as, between intracranial pressure and mean arterial pressure was not significant.   Conclusions: There is no anatomical and pathophysiological basis for the statement that intraocular pressure can be used as an indirect estimator of intracranial pressure.
