Abstract :

Schizophrenia and obsessive compulsive disorder become our concern because they have an important neurobiological and phenomenological. Westphal considered obsessive-compulsive symptoms to be either a prodrome or an integral part of schizophrenia. A major challenge in the identification of obsessive compulsive phenomena in schizophrenia is the differentiation of obsessions from delusions and/or formal thought disorders that are characteristics of schizophrenia, and of compulsions from schizophrenic mannerisms and stereotypic behavior. In the absence of evidence-based data, a careful evaluation of the potential risks and benefits of pharmacotherapy in schizo-obsessive patients is required. Due to the risks of psychotic exacerbation or increased aggresive behavior, anti-obsessive agents should be initiated for obsessive compulsive symptoms only in neuroleptic-stabilized patients. Patients with obsessive compulsive symptoms had earlier age of onset of psychosis and a greater number of hospitalizations than patients without obsessive compulsive symptoms.
