Antenatal care (ANC) and delivery with health personnel became the most important indicators for reproductive health program. The utilization of maternal health program is related to accessibility to health services, and it depends upon physical, economical and social aspects of customers and vice-versa the health-provider services. Kabupaten Tangerang has experienced high accessibility to ANC first visit (K1), but relatively low to the fourth visit (K4) as well as delivery help by health personnel. This phenomenon raised a questioned why it happened, since the access was there already? This research tried to find out the answer behind the phenomenon. Three villages were chosen based on the low ANC coverage and also low delivery by health personnel. Most informants mentioned that they did not realize they were pregnant, until they felt unwell and visited a midwife or a traditional birth attendant (TBA). Although they visited midwives for ANC, but most informants preferred to have delivery at home and asked a TBA to assist them and it was due to social and economic aspects
