Abstract :
Cooperative is the pillar of the Indonesian economy, the cooperative has the characteristics of mutual cooperation and common progress, the cooperative was also able to survive in the era of globalization. The purpose of this research was to determine the factors that cause the death of co-operatives in the city of Blitar. This is qualitative and explorative research which inductive approach. Department of Cooperatives and Small Micro Economic’s Blitar cityamount of the 312 cooperatives which 24% or 75 cooperatives in Blitar City declared inactive or near to death. The results of the study by comparing the performance of cooperatives based on the quality of the organization's performance with four indicators of inputs (input), output (output), outcomes (results) and impact (impact). There is a 24% or 75 units a cooperative of 312 cooperatives in the Department of Cooperatives and SMEs Kota Blitar inactive or near to death, consisting of 38 types of business cooperatives and 37 other types of cooperatives. Factors causing of inactive or near to death: (1) inputs factor (input) is: difficult to obtain raw materials, lack of capital, lack of skilled manpower, resources are limited. (2) output factors (output) are: difficulty inmarketing, limited marketing area, many competitors of similar products, the existence of substitute goods, technology is simple, operation is less known brand. (3)results factors (outcomes) are: without financial records (financial statements), limited skill to manage finances (financial management), the use of traditional technology (operations management), limited skill to manage product (production management), limited skill to make a budget (planning), limited skill to knowledge (skills). (4)impact factors (impact) the input (input is available, the input is available), the output (according to the market and not according to the market), the outcomes (the production process, technology, quality), cause the purpose of the cooperative is financially is to improve operating results, can not be achieved due to internal and external factors that generally occurs due to limited resources and markets are less supportive
